Fraternity & Sorority Affairs
This form is to be completed by the chapter's philanthropy chair or president by the first
Friday of each month for the previous month's activities. If chapter members participate in an on-going activity,
please submit the total hours and dollars as one activity at the end of each month. Only submitted forms will be
considered for Accreditation purposes, any Office of Fraternity and Sorority Affairs recognition, or reports to
Lehigh University constituents.
Philanthropic activities identify a need and raise monetary funds or donated items to
dispense for humanitarian purposes (i.e. Relay for Life, Adopt-a-Family). Please note that donating blood would
be considered community service. Only time spent participating in the activity should be calculated towards the
