The Davis Vision program, offered through Highmark Blue Shield, is part of each medical plan offered by Lehigh. Davis Vision has over 23,400 vision care providers and optical supplier locations across the United States. There are over 2,000 providers and suppliers in Pennsylvania, with more than 141 in the Lehigh Valley area.

Each individual covered by Lehigh's medical insurance plans can receive the services or supplies listed in this chart once every twelve (12) months.

By using a network provider/supplier, the services or materials received will be covered in full by the plan or covered from the first dollar to the maximum level. If you are using a non-participating supplier or provider, there is a specific level of reimbursement of each service or supply that the program will cover.

You or your dependents can receive any of the covered services and products as needed more often than once in every twelve (12) months if you use network providers or suppliers for all materials and services. When a network provider or supplier is used for such additional covered services or supplies, covered employees and dependents receive a 20 percent discount off of the provider’s standard charge.

If you and/or your dependents wear both contact lenses and glasses, only one will be covered at the in network benefit level each year. The other will be covered at the out-of-network level when a network provider or supplier is used for all materials and services.
