I) How to Write a Research Report
1. Select a topic
a. The topic should be sufficiently narrow to be covered in a limited report, sufficiently
broad that you can locate enough information on the topic. For example, do not select a brain disease that is so rare there is not much research or information on the subject.
b. Select at least two possible topics before beginning your research.
2. Locate Sources
a. Sources you might use include:
i. Library books
ii. Magazines
iii. Internet
iv. Charts
v. Newspapers
vi. Interviews
3. Check the table of contents and index of each reference
a. Skim the material to find pages that discuss the topic
4. Make choices about the topic according to:
a. Reading ability
b. Level of material
c. Time available for the project
5. Prepare notes for reference material
a. Read carefully, take notes on the main ideas and facts
i. Write phrases in your own words
b. Write at the top of the notes:
i. Title
ii. Author
iii. Publisher
iv. Place and date of publication
v. Page number
c. This is important information you will need later for the bibliography
6. Prepare an outline

III) Case History Report
The assignment is to choose any disorder of the cardiovascular system and develop a Case History Study using the following template and rubric as a guideline. Note this is a realistic assignment. If you enter the health care professions, you may have to develop case histories frequently.

Presenting Problem:
|
Reasons why the patient went to the doctor. This is basically the symptoms t realize that a given patient may not exhibit all the symptoms of a disorder.

History:
|
How long has the patient had the symptoms? Include any previous treatments and how the patient responded, anything that makes the symptoms worse or better, life style information that may be pertinent (e.g. smoker, poor diet, exercises regularly, etc.)
|
Family History:
|
If the disease is inherited this section will be very important. Some disorders are not genetic, but heredity may predispose a person to a disease. Do any other family members in recent generations have the disease (include siblings)? Has any family member died from the disease?
|
Exam Results:
|
General observations made by the physician during an office visit. Information like height, weight, heart sounds, blood pressure, eyes and ears, etc.
|
Laboratory Studies:
|
This could include blood work, x-rays, urinalysis, etc. Note that blood work and urinalysis are routine procedures that are nearly always done. If they would show nothing for your disease, report them as normal (medical professional use the term unremarkable) Explain any other tests you might perform.

Diagnosis:
|
Name the disease
|
Physiology of the Disorder:
|
Explain the anatomy and physiology that underlies this disorder.
|
Course of Treatment:
|
What can be done? Consider both immediate and long term treatments, hospitalization, surgery, occupational or physical therapy, medication, counseling, etc. Include probable length of treatment.

Prognosis:
|
Explain the long-term expectations. What could happen to this patient in the future?
