Community Service Report

This form is to be completed by the chapter's community service chair or president by the first Friday of each month for the previous month's activities. If chapter members participate in an on-going activity, please submit the total hours as one activity at the end of each month. Only submitted forms will be considered for Accreditation purposes, any Office of Fraternity and Sorority Affairs recognition, or reports to Lehigh University constituents.

Community Service hours are the "hands-on" time spent engaging in action to meet the needs of others to improve, support and better the surrounding community as a whole, helping to foster social change (i.e. M.O.O.V, Spooktacular, soup kitchens). According to this definition, donating blood would be considered community service.

If any funds or items were also donated as part of an activity, please submit a philanthropy reporting form as well.
