ARTWORK APPROVAL INFORMATION SHEET

When making a request to use departmental and/or University identity marks, the following process should be used:

1.   Previous to placing any order, complete LEHIGH ATHLETICS ARTWORK APPROVAL REQUEST FORM.
2.   Submit form to Director of Equipment Operations, Tyler Osler
3.   If not already on file, submit sample artwork from requested vendor to Director of Equipment Operations, Tyler Osler
4.   If all information is completed and sample artwork is in hand, approval will be made within 24 hours.
      *Approval may be delayed without sample artwork.
5.   Following request approval, order may be placed with specific vendor.

Contact Information

Date of Request: fill in the date request is being made for artwork approval

Contact Name: fill in the name of the person making the artwork request

Contact Number: fill in the phone number of the person making the artwork request

Specific Use of Identity Mark(s)

Mark(s) Requested: list the specific marks(logos) that are being requested for use.

Mark Location(s): list the locations where the marks will be placed on the specific items

Mark Color(s): is mark one color only or full color

Item(s) Description: fully list the item(s) that the identity mark is to be placed on and its anticipated use

Item Material(s): list the type of material the marks will be placed on, this includes both soft and hard goods

Item Color(s): list the color of the item that the marks will be placed on

Vendor Information

Vendor Name: list the company name of the vendor you wish to use

Vendor Contact: list any individual at the vendor that can answer questions relating to artwork use

Vendor Number: list the phone number of the contact person at the vendor

Authorization

Requestor: signature of person making artwork request agreeing that all information has been provided

Director of Equipment Operation: to be signed by Equipment Director indicating approval of request
