The Lehigh University Health and Wellness Center has arranged for H1N1 swine flu vaccination clinics to be held Wednesday, November 18th, 2009 and in Lamberton Hall’s Great Room, FOR FACULTY & STAFF ONLY.

for those who wish to be vaccinated

Those who wish to be vaccinated against H1N1 can help make the process go smoothly by:

• Reviewing the VIS (vaccine information sheet)

• Printing out and completing the consent form in advance and bringing it with you to Lamberton

• Bringing Documentation of underlying conditions for those 25 and older.
