Hard-copies of the form are also available in Academic Life & Student Transitions (31 Williams Hall, Suite 390) or your Undergraduate Associate Dean’s Office.

General Information

The SOS Committee meets regularly (every other Thursday) throughout the semester and summer; please refer to the meeting schedule listed below. Petitions are due in your Undergraduate Associate Dean’s Office by the Monday (4:45 p.m.) before each SOS meeting; only completed petitions will be accepted to include all of the required signatures and relevant documentation. Petitions submitted after this deadline will not be considered until the following meeting. *All petition documents should be one-sided.

Information Regarding Make-up Final Examinations

All final examinations are expected to be taken as scheduled. Please complete this form to request consideration for an exception to take a make-up final examination due to extenuating circumstances: Request for Make-up Final Exam
