The purpose of the Absence Reporting Form is to document and report a student absence to faculty members.
The student will submit the form (online) with the required documentation and then they will receive email confirmation for the absence. It is then, the student’s responsibility to forward the absence letter (email confirmation) to the appropriate faculty members that require absence documentation.
Please Note: Students should review the course syllabus and faculty make up policies, as this reporting form will not over ride those policies.
